Provide the following information:
1.- Name and information of the person who is reporting (phone, cell phone, email, address).
2.- The initials, first and last names of the person presenting the adverse reaction, sex and age.
3.- Adverse reaction (indicate if there was any skin discomfort, for example, irritation, redness, etc.)
4.- Name of the medicine by Compañía Internacional de Comercio suspected of having caused the adverse reaction.
